The Secretin Test Clues to the diagnosis of Zollinger-Ellison syndrome: In one type of gastrin stimulation test, the patient is administered secretin, which normally inhibits the release of gastrin. Significantly elevated levels of gastrin after the test may suggest a gastrinoma, in which gastrin secretion is paradoxically increased by secretin due to abnormal secretin receptors. Other conditions with high gastrin, like atrophic gastritis, do not exhibit an increase in gastrin levels using the same test. Insulin and Glucagon Physiology Beta cells, which make up most of the islet cells, secrete insulin. Insulin facilitates the entry of glucose into cells for energy or for storage and lowers blood glucose levels in the process. In liver and muscle cells, glucose is stored as glycogen (glycogenesis), a complex carbohydrate. Glucose can also be broken down (glycolysis) and stored in fat cells as triglycerides. Or, the glucose can be broken down (glycolysis) to acetyl CoA and immediately used for energy in the mitochondrial Krebs cycle, which generates energy rich ATP molecules. Adrenal Inusfficiency Addison’s Disease is a disorder of the adrenal cortex that arises from an undersecretion of cortisol, aldosterone and adrenal cortical androgens. Different symptoms result from the deficiency of cortisol, aldosterone, or s-ex hormones. In secondary adrenal insufficiency, the pituitary gland doesn’t secrete enough ACTH. This does not affect aldosterone secretion, which is mainly controlled by the renin-angiotensin-aldosterone system. As a result, while cortisol deficiency may be present, hyponatremia, hyperkalemia, and dehydration are less common, since aldosterone is not compromised.
